Zombies wander rather than hunt in a straight line, so the safer play is usually circling wide around a loot spot to see what's patrolling near it before committing to the stop, rather than beelining for the nearest item and dealing with whatever shows up.

Why did a zombie reach me while I was in the middle of looting an item?

Interacting with an item takes a moment where your character stays in place, so a zombie that was out of range when you started can close the distance during that pause — checking what is nearby before committing to a loot stop avoids it.

Do the zombies actively hunt me, or just wander the map?

They mostly wander rather than track you directly, so circling wide around a loot spot to see what is patrolling nearby before moving in tends to be safer than walking straight toward the item and reacting to whatever happens to be close.
