Why do I die almost instantly once zombies get on both sides of me?

Contact damage from multiple zombies stacks fast, and shooting one at a time doesn't clear a surround quickly enough — Shift-dash is built specifically for that situation, since it moves you out of contact range in a way that firing in place can't.

Is it better to use a grenade on a single tough zombie or wait for a group?

Save it for a cluster — the blast damages everything caught in its radius at once, so spending it on one target wastes most of its value compared to using it the moment several zombies bunch up around a chokepoint.
